Who is responsible?

What we collect
The form records the parent’s first name, email, child’s age, language choice, optional observation category, submission time, consent choices and their wording version. During an active giveaway, it also records the eligibility declaration, rules version and entry status. Do not provide the child’s name, diagnosis, health records or free-text clinical details.
Why and your choices
Information is used to process the guide request, provide download access, administer eligible contest entries and maintain consent records. Optional speech observations help understand the request; they are not used for advertising audiences or winner selection. Ongoing marketing is optional and separate. Declining it does not affect guide access or contest eligibility. The form is for adults, not children.
Hosting and access
This website is hosted through ChatGPT Sites and Cloudflare infrastructure. Form records are stored server-side in the site database and can be exported only through the owner-authorized endpoint. Processing may involve providers outside Quebec or Canada. The practice must complete its required privacy assessment and confirm provider arrangements before public collection. Jane separately handles clinical bookings and intake; its own privacy terms apply there. The website does not email the guide until an email service is connected.
Retention and rights
Before public launch, the practice will finalize and publish its retention schedule. Proposed schedule: review guide-only requests without marketing consent for deletion within 90 days; retain contest administration records until 90 days after prize completion, subject to legal requirements; retain opted-in contact details until withdrawal or an applicable retention limit. This is a draft policy requiring the practice’s confirmation, not an automatic deletion claim. Contact Allixia to request access, correction or deletion, subject to lawful retention obligations.
Measurement without clinical details
The guide page records aggregate events: page view, form started, successful submission, PDF download, consultation click and booking-page click. Event records contain only the event name, page language and timestamp, without form fields, email, age, speech observation, IP address or advertising identifiers. No Meta pixel or third-party advertising tracker has been added. The form uses a random download-access token, stored in session storage after success so the download remains available on reload.
Educational resource
Downloading the guide does not create a therapist-client relationship, assessment, screening or diagnosis. Clinical suitability and consent are established separately with Allixia. Last updated: October 26, 2026.
